Anthony Wayne Local Schools has partnered with FinalForms, an online forms and data management service for athletic participation documentation. Please create an account to provide your child's information prior to the start of practice.

All student-athletes must have a current athletic physical on file prior to beginning practice. Once a physical has been completed by a physician, the form can be returned to the Junior High Office or the High School Athletic Office.
