Anthony Wayne Local Schools partners with FinalForms, an online data management service for athletic participation. Please create an account to provide your child's information prior to the start of practice.

All student-athletes must have a current athletic physical on file prior to beginning practice. Once a physical has been completed by a physician, the form can be returned to the Junior High Office or the High School Athletic Office.
